Dear Valued Patient,

It is with mixed emotions that I announce my retirement after 48 years of practicing medicine. Caring for you, my patients, has been the most rewarding part of my career. I would like to take this opportunity to thank you for your years of loyalty and patronage. It has been my privilege to serve you.

I will be retiring and closing my medical practice on November 1, 2022.

As I will no longer be able to continue providing care, or renewing your prescriptions, I am pleased to introduce you to the team of podiatrists at UP Medical who come with years of experience in general podiatry, complex wound care, and foot & ankle surgery. The team at UP Medical is committed to providing the high level of care that my patients have come to expect. There will be a continuum of care in services we have provided for years including, orthotics, laser nail care and pain therapy treatment. This also includes our workers compensation patients for their orthotics.

Please accept my invitation to make an appointment with the podiatrists at UP Medical and reach out to their office if you have any questions or concerns about the transfer of care. I know that you will be in good hands at UP Medical and we have included their podiatrist bios, so you can know who will be there to continue your care.

If you choose to not continue your care with UP Medical, we recommend you arrange to place yourself under the care of another podiatrist as soon as possible. After November 1, 2022, all patient records will be sent to UP Medical to be the custodian of medical records. If you need a copy of your medical records, please request them from UP Medical by calling their main phone line 323-438-0483, option #4, or by fax 760-951-5498.
